How to use this template
This section will give you an idea of what information to include in this template as well as a sample template on the next page which has been filled out. It is intended to help you think through a process that can be used in your church. Whatever process you decide to use will be unique to your place of ministry. Definition:
Come up with a common terminology or understanding that everyone in your church can remember. Write it in a way that reflects your church and your setting. Some questions to consider. How will your church define what it means to be a disciple of Jesus Christ? What will it look like when someone is a fully formed disciple?
Process Plan:
This is a place to define the markers that identify where a person is at as a disciple. Define the steps or stages of growth as a disciple and how your church plans to communicate them. Some questions to consider. How will you identify where a person is at on the path to becoming a mature disciple? What process do you plan to follow for helping people grow as disciples?
Tools:
The Bible is key, and you may only decide to use Scripture alone as your core discipleship study tool. Or you may find that there are other materials and study guides you want to use as a supplement. Let us know what materials you plan to use.
Disciplemakers and Dates:
Identify one or more key leaders who will be at the heart of your initial discipleship process and a planned date to start a discipleship group.

Definition of a disciple:
A disciple is one who seeks to follow Jesus daily, studies the Bible regularly, serves others intentionally and shares what he has learned with others in order to make more disciples.
Process Plan:
Our process and definition are based around four stages of discipleship: a new believer (seeks), a growing believer (studies), a maturing believer (serves) and a disciplemaking believer (shares). We will use the Disciplemaker Stages app as a common touchpoint for all of those who are doing discipleship groups of 3 to 5 people per group. I will be starting the process by forming a group of people initially.
